Underwood was drafted by the Chicago Cubs in the second round of the 2012 Major League Baseball draft out of Alan C. Pope High School in Marietta, Georgia.
Although Underwood had the intention to play college baseball for the Georgia Bulldogs, he instead signed with the Cubs and made his professional debut that same year with the Arizona League Cubs.
In the year 2013, Underwood spent time with with the Boise Hawks and later in the next year with the Kane County Cougars.
Underwood was promoted to the Myrtle Beach Pelicans, in the year 2015 posting a 2.58 ERA in 73.1 innings. He spent the next year with the Tennessee Smokies, South Bend Cubs, and Myrtle Beach Pelicans, posting a 0–6 record with a 4.32 ERA. After the season, he played for the Mesa Solar Sox of the Arizona Fall League and was added to the Cubs 40-man roster before spending time with the Tennessee Smokies.
After starting the 2018 season with the Iowa Cubs, he was promoted to the major leagues and made his first start against the Los Angeles Dodgers. He was optioned back to Iowa after the game and spent the remainder of the year in Iowa.

After 8½ seasons in the minors, Duane Underwood Jr. showed Tuesday night there might be a place for him on the Cubs major-league roster in 2020.
Underwood, who transitioned from starter to reliever at Triple-A Iowa earlier this season, showed the talent that made him a second-round pick in the 2012 draft.
Making his 2019 Cubs debut, Underwood struck out six consecutive batters to tie a franchise record for a reliever shared by Juan Cruz in 2003 and Hall of Fame closer Bruce Sutter in 1977.
“That was impressive,” said starting pitcher Jon Lester, who allowed 11 runs in four innings in the 11-4 loss. “He probably should have started the game.”
He displayed a fastball Tuesday night that topped out at 97 mph, and his changeup sank effectively.
“If we can replicate that — being in control of your emotions and throwing strikes — I’ll take it,” manager Joe Maddon said.
Underwood is out of minor-league options, so next spring he must make the major-league roster or be exposed to waivers. Relievers Steve Cishek, Brandon Kintzler, Brandon Morrow, David Phelps, and Derek Holland are potential free agents after this season, so Underwood could enhance his status for 2020 by taking advantage of opportunities.
Underwood dropped 25 pounds and changed his diet five years ago, and he has overcome arm injuries in previous seasons that stagnated his development.
“I was happy to get in the game and show the team what I’ve got,” said Underwood, who allowed one run on four hits against the Dodgers on June 25, 2018, in his only previous major-league appearance. “Hopefully I showed them enough, and whatever I need to do to help the team is my only goal.”
